LIBRARY INSTRUCTIONAL REQUEST FORM (FACULTY ONLY)

INSTRUCTIONS: Choose a specific date. Try to pick a date after the assignment has been given (if there's a research paper, after a thesis statement has been developed). Please be sure also to list at least two alternative dates and make sure this form is submitted at least 14 working days in advance. Please make sure all the information requested is provided otherwise it will be difficult to contact you or to know what you need. You will be contacted via e-mail or by phone to confirm the date/time you have requested, or to schedule an alternate date.
